Employee Resignation Process

Exit Interviews

Part of Uplift Education’s exiting process is to conduct an exit session with all departing employees via our confidential online exit interview questionnaire. After you’ve completed the online survey, you have the option to request a face-to-face or phone interview.

Please take a few minutes to share your valuable feedback about your work experience at Uplift Education.

Requesting Service Records

Service Record Requests can be submitted to Human Resources for processing. Service Records are official State documents that serve as verification of a full-time employee’s service and State personal leave information. Typically, record request take approximately two (2) weeks to process.

Salary Continuation

School-year employees who work through the last day of school will continue to be paid through July 2016. Year-round employees will be paid through their last day of employment.

State/local and remaining vacation days, if any, will not be paid.

Benefit Plan Continuation

If you are a school year employee: Coverage under the Uplift Education employee benefit plans will end on July 31, 2016. You may elect to continue your medical plan coverage through August 31, 2016 at the subsidized rate, but must request to do so with Human Resources by July 15, 2016. If premiums for medical insurance are owed, please send/bring payment to Uplift CMO by Friday July 25, 2016.

If you are a year round employee: Coverage under the Uplift Education employee benefit plans will end the last day of the last month in which you work. You may elect to continue your medical plan coverage through August 31, 2016 at the subsidized rate, but must request to do so with Human Resources before your last day. If premiums for medical insurance are owed, please send/bring payment to Uplift CMO before the 25th of each month.

Beyond August 31, your rights to continue certain coverage under COBRA (for up to 18 months) will be provided to you under separate cover from Health Care Services Corporation for medical coverage (888-541-7107), and from CobraOne (Uplift’s third-party administrator) for dental, vision and flexible spending (877-230-0970).

Refund or Rollover of Retirement Funds

For questions regarding your retirement benefits through the Texas Teacher Retirement System (TRS), you may contact them at 800-223-8778 or visit http://www.trs.state.tx.us. Should you elect to withdraw/rollover your TRS funds from your account, you can request the refund through the “MyTRS” option from the TRS website.

Continuation of Other Benefits (not subject to COBRA)
